Efficacy of endoscopic surgery for early stage cervical cancer patients with adverse factors at surgery. Patients with adverse factors at surgery are considered poor candidates for surgery. They have an increased risk of morbidity from surgery and recurrence. However, surgery is the only curative treatment option for patients with early stage cervical cancer (FIGO stages IA2-IB1). Our aim was to determine the efficacy of endoscopic surgery for patients with adverse factors at surgery. A retrospective analysis of the records of 102 patients with cervical cancer who underwent endoscopic surgery was carried out. Clinical data, pathological findings, and treatment outcome were obtained from the medical records. Survival rates were calculated using the Kaplan-Meier method. The patients were divided into two groups: a "conventional treatment group" (n = 42) and a "endoscopic surgery group" (n = 60). The median follow-up period was 30 months (range, 6-72 months). The overall 3-year recurrence-free survival rate and overall survival rate were 91.2 and 95.5 %, respectively. No recurrences occurred in the endoscopic surgery group, whereas nine (9.0 %) recurrences occurred in the conventional treatment group (p = 0.002). The number of cases of pelvic lymph node metastasis was significantly lower in the endoscopic surgery group (3.3 %) than in the conventional treatment group (33.3 %) (p = 0.000). The post-operative morbidity was relatively high in the conventional treatment group, but there was no difference in the morbidity between the two groups.
